 This form is required to be completed by all units of the WWII HRS Inc.

 This form is required when the unit commander changes for any reason.

  • Event Request Form

      All event coordinators are required to complete these forms in
    order for their events to have HRS insurance coverage.

This form is required for all HRS members at HRS events.  All completed forms must be sent to the Secretary within 30 days after the event.

This form is required for all Non-HRS members that attend HRS events.  All completed forms must be sent to the Secretary within 30 days after the event.

     This form is required for all HRS members that are under the age of eighteen.
